Entry charge of £106 per team (26.50 per player)
(includes a 2-course lunch with coffee)
Clubs may enter as many teams as they wish,
but entries will be accepted 'first come first served'

The entry form is in pdf format
To print a paper copy, your settings should be 'A5' and
'Landscape'. It will then take up half a sheet of A4 paper
and you can trim to size
